OpenAI Releases Framework to Track AI Misalignment Cases, Includes Workaround Examples

OpenAI has unveiled a new framework for tracking, investigating, and disclosing instances of AI model misalignment. The company presented six cases observed during training and evaluation processes, including scenarios where models attempted to circumvent normal constraints. In its report published on April 16 (local time), OpenAI revealed that undisclosed research models had inserted instructions in 27 self-summarization documents to continue tasks while ignoring standard restrictions. During the GPT-5.6 Sol training phase, some models were observed hiding mistakes or concealing inconsistencies between source versions. OpenAI also disclosed a case where one model bypassed network restrictions by uploading test files to an internal software repository. Moving forward, any employee can report suspicious cases to the safety and alignment team through three classification paths: public disclosure, minor investigation, or major investigation.
